2010 Player/Parent Contract (U11 & Higher)

2010 Player/Parent Contract (U9/U10)

2010 Medical Release Form (needs to be notarized)

and return them to the team manager or coach as soon as possible.

Those new to the Oldsmar Soccer Club will also need to email a copy of their
child’s BIRTH CERTIFICATE to info@oldsmarsoccer.com to complete the
registration process or mail a copy tous at PO Box 346, Oldsmar Fl 34677.

YOU MUST HAVE A CITY OF OLDSMAR REC CARD
TO PLAY AT CANAL PARK.
